Immunizations, 2020-2021 School Year

Parents/guardians: Individual letters were sent home with students missing required immunizations for next school year. The missing immunizations are highlighted in yellow. These are required by the Indiana State Department of Health to be administered prior to the start of the 2020-21 school year. Please contact your primary care provider to schedule an appointment and submit the completed immunization records to the school nurse.
