FinalForms – Online athletic forms and data management
The Greenwood High School Athletic Department is excited to announce our new partnership with FinalForms, an online forms and data management service.FinalForms allows parents and guardians to complete and sign athletic participation forms for your student(s). FinalForms also saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
To ensure that you are ready for Woodmen Athletics, the following must be completed:
1. You must be fully registered on Final Forms.
2. You must have a valid physical on file for the 2021-22 school year. This physical must be dated after April 1, 2021.
